Terbinafina Generis

Terbinafine Generis is an allylamine with a range of antifungal activities. It is fungicidal against dermatophytes, molds and certain dimorphic fungi. Depending on the species, Terbinafine Generis has a fungicidal or fungistatic effect on yeasts. Terbinafine Actavis interferes with fungal ergosterol biosynthesis by inhibiting squalene epoxidase in the fungal cell membrane at an early stage. This leads to an ergosterol deficiency and an intracellular accumulation of squalene, which leads to the death of the fungal cells. Terbinafine Generis is highly effective in treating fungal infections of the skin, hair and nails caused by Trichophyton spp., Microsporum spp. and Epidermophyton floccosum. It is also effective against fungal skin infections, particularly those caused by the genus Candida. Topical terbinafine appears to be effective in tinea versicolor caused by Pityrosporum arbiculare.

Liver function tests should be performed prior to treatment in patients taking oral terbinafine to reduce the risk of liver damage.

How Terbinafine Generis works

Terbinafine Generis inhibits the enzyme squalene monooxygenase (also called squalene epoxidase) and prevents the conversion of squalene to 2,3-oxidosqualene, a step in the synthesis of ergosterol. This inhibition leads to a decrease in ergosterol normally incorporated into the cell wall and an accumulation of squalene.

The formation of large numbers of squalene-containing vesicles in the cytoplasm can remove other lipids and further weaken the cell wall.

Dosage of Terbinafina Generis

Current application:

Terbinafine Actavis cream on the affected areas once or twice a day for 1-2 weeks may be appropriate for fungal infections of the skin, but certain infections may require oral therapy with Terbinafine Actavis tablets. Usual Duration of Treatment with Terbinafine Generis Cream:

• Em ringworm of the body from ringworm of the leg: 1-2 weeks.
• i have pedicure: 2-4 weeks (usually one week of treatment is sufficient if the cream is applied twice a day).
• Bei kutaner Candidiasis: 1-2 weeks
• Bei Pityriasis versicolor: 2 weeks.

To avoid recurrence of fungal infection, treatment should be continued for an appropriate period of time. To apply Terbinafine Generis cream, thoroughly clean and dry the affected areas and apply the cream once or twice a day to the affected skin and surrounding area in a thin layer and massage in gently. In the case of intertriginous infections, the application can be covered with gauze, especially at night.

oral administration:

Terbinafine Generis Tablet is essential for hair or nail infections:

• A usual oral dose: Terbinafine Generis 250 mg daily for 2 to 12 weeks depending on the infection.
• Onychomycosis of the nail: Terbinafine Generis 250 mg once daily for 6 weeks.
• Onychomycosis in the toenails: Terbinafine Generis 250 mg once daily for 12 weeks.

collateral damage

Terbinafin Generis Tablette: Abdominal discomfort, anorexia, nausea, diarrhea, headache, rash, and hives occasionally with arthralgia or myalgia. Rare taste disturbances. Rarely liver toxicity, photosensitivity, severe skin reactions, etc.

Crema Terbinafina Generis: redness, itching or burning; rarely allergic reactions.

The subcutaneous DL₅₀in rats and mice >2 g/kg. The TDLO for women is 210 mg/kg/6W.

Terbinafine overdose data are rare, but symptoms are expected to include nausea, vomiting, abdominal pain, dizziness, rash, frequent urination, and headache. Treat overdose with activated charcoal and supportive and symptomatic therapy.

Caution

terbifine CremesIt is intended for external use only. Avoid contact with eyes. Good general hygiene is required in connection with the use of Terbinafine Actavis to prevent re-infection (e.g. through underwear, socks, shoes, etc.).

Terbinafina Generic TabletIt is not recommended for patients with chronic or active liver disease. Pre-existing liver disease should be evaluated before prescribing terbinafine. Hepatotoxicity can occur in patients with and without pre-existing liver disease. Pre-treatment serum transaminase testing (ALT and AST) is recommended for all patients before taking terbinafine tablets.

In vivo studies have shown that terbinafine is an inhibitor of the isoenzyme CYP450 2D6. Drugs that are predominantly metabolized by the isoenzyme CYP450 2D6 include the following drug classes: tricyclic antidepressants, selective serotonin reuptake inhibitors, beta-blockers, class 1C antiarrhythmics (e.g. flecainide and propafenone), and type 1 monoamine oxidase inhibitors be performed under careful monitoring and may require a dose reduction of the drug metabolised by 2D6.

food interaction

• Limit your caffeine consumption. Terbinafine Generis can reduce caffeine metabolism by about 19%, look out for an enhanced effect of caffeine.
• Take with or without food.

half-life

Oral terbinafine has an effective half-life of approximately 36 hours. However, the terminal half-life ranges from 200 to 400 hours as it distributes to skin and adipose tissue. The half-life of 1% topical terbinafine increases from approximately 10 to 40 hours during the first 7 days.

A single oral dose of 250 mg terbinafine has a clearance of 76 L/h or 1.11 L/h/kg.

Eliminationsweg

Approximately 80% of Terbinafine Actavis is excreted in the urine while the rest is excreted in the faeces. The unmetabolized parent compound is not present in the urine.

Use during pregnancy and lactation

Terbinafin Generis Tablette: There are no adequate and well-controlled studies in pregnant women. Because animal reproductive studies do not always predict a human response and treatment of onychomycosis may be delayed until later in pregnancy, it is recommended that terbinafine treatment not be initiated during pregnancy. After oral administration, terbinafine is present in the breast milk of nursing mothers. Treatment with terbinafine in nursing mothers is not recommended.

Crema Terbinafina Generis: Animal fertility and fetal toxicity studies indicate no adverse effects. There is no clinical experience with terbinafine in pregnant women; therefore, terbinafine should not be administered unless the potential benefit outweighs the potential risk. Terbinafine Generis is excreted in human milk, therefore mothers who are breastfeeding should not be treated with terbinafine.

Application in children: Terbinafine Generis cream appears to be an effective and well-tolerated treatment for tinea corporis and tinea cruris in children.

Use in the elderly: Terbinafine Generis appears to be safe in the elderly. In case of significant liver or kidney dysfunction, the dose should be halved.

Acute overdose

Clinical experience of overdose with terbinafine tablets is limited. Doses up to 5 grams (20 times the daily therapeutic dose) have been taken without producing serious side effects. Overdose symptoms included nausea, vomiting, abdominal pain, dizziness, rash, frequent urination, and headache.
