1. Name of drug

Canesten HC cream

2. Qualitative and quantitative composition

Each gram of cream contains 10 mg of clotrimazole and 11.2 mg of hydrocortisone acetate (equivalent to 10 mg of hydrocortisone).

Excipients with known action:Ketostearyl alcohol 100 mg in each gram of cream, benzyl alcohol 20 mg in each gram of cream.

4. Clinical specialty
4.1 Therapeutic indications

Canesten HC is indicated for the treatment of the following skin infections, with accompanying signs of inflammation, e.g. itching, need quick relief:

a) All dermatophytes caused by dermatophytes (eg Trichophyton species), molds and other fungi.

b) All dermatomycosis caused by yeasts (Candida species).

c) Skin diseases with secondary infection by these fungi.

d) Treatment of diaper rash caused by Candida albicans. Candida vulvitis, Candida balanitis and Candida intertrigo.

4.2 Dosage and method of administration

dosage:

Canesten HC Cream should be applied in a thin, even layer to the affected area twice a day and rubbed in gently. Treatment should last a maximum of 7 days.

The total daily dose of 10 mg of cream per kilogram of body weight should not be exceeded. For an adult weighing 50 kg, the maximum daily dose is 500 mg of cream, which corresponds to approximately 2 cm of cream, which should be divided into 2 applications per day.

There is no separate dosage regimen for the elderly and the young. However, long-term treatment on large areas of skin, especially in infants and children, should be avoided.

Duration of treatment:

If acute symptoms subside after about 7 days, but treatment is still necessary, this can be done with a non-steroidal preparation intended for this purpose.

4.3 Contraindications

- Hypersensitivity to the active substances or to any of the excipients mentioned in paragraph 6.1.

- Apply to damaged skin

- Skin diseases (e.g.

- Any untreated bacterial skin disease

- Viral skin diseases (e.g. herpes simplex, chicken pox, shingles, etc.)

- skin reactions after vaccination.

4.4 Special warnings and precautions for use

Due to the corticosteroid content, Canesten HC should not be used:

• on large surfaces (over 5 - 10% of the body surface).

• in long-term continuous treatment.

• under occlusive dressings (such as diapers and bandages).

These restrictions apply in particular to:

• Infants where the diaper can act as an occlusive dressing and increase systemic absorption.

• Infants and children who may experience increased systemic absorption leading to adrenal suppression.

This product contains ketostearyl alcohol which may cause local skin reactions (eg contact dermatitis). The cream also contains benzyl alcohol which can cause allergic reactions and slight local irritation.

Do not smoke and keep away from open flames - Risk of severe burns. Fabrics (clothing, bedding, bandages, etc.) that have come into contact with this product burn more easily and are a serious fire hazard. Washing clothes and bedding can reduce product build-up but not remove it completely.

Visual disturbances:

Visual disturbances may occur with the use of systemic and topical corticosteroids. If patients experience symptoms such as blurred vision or other visual disturbances, they should be referred to an ophthalmologist for evaluation of possible causes, which may include cataracts, glaucoma, or rare conditions such as central serous retinopathy (CSCR) that have been reported following systemic and topical corticosteroids .

4.5 Interactions with other drugs and other forms of interaction

Laboratory tests have shown that this product can damage latex contraceptives when used together. Consequently, the effectiveness of such contraceptives may be reduced. So far, this has not been reflected in clinical practice.

4.6 Fertility, pregnancy and lactation

Fertility:

No human studies have been performed on the effects of clotrimazole on fertility. However, animal studies have shown no effect on fertility. No data are available on the effect of topical hydrocortisone.

Pregnancy:

There are limited data from the use of clotrimazole or hydrocortisone in pregnant women. Animal studies with clotrimazole and corticosteroids have shown reproductive toxicity (see section 5.3). At low systemic exposure to clotrimazole and hydrocortisone after topical treatment, reproductive toxicity is not expected.

Canesten HC cream can be used during pregnancy, but only under the supervision of a doctor or midwife. As a precautionary measure, it is recommended to refrain from using the cream for extended periods of time during pregnancy.

Lactation:

Available pharmacodynamic/toxicological data in animals showed excretion of clotrimazole/metabolites in milk after intravenous administration (see section 5.3).

No data are available for hydrocortisone, but topical hydrocortisone is unlikely to cause systemic effects due to poor percutaneous penetration. However, skin absorption may be increased under certain conditions, such as the use of an occlusive dressing, the degree of skin damage, and the size of the treated area.

Risk to the infant cannot be excluded. A decision should be made whether to discontinue breast-feeding or to discontinue treatment with Canesten HC taking into account the benefit of breast-feeding for the child and the benefit of treatment for the woman.

4.7 Effects on ability to drive and use machines

Canesten HC has no effect on the ability to drive and use machines.

4.8 Adverse Effects

Since the reported side effects are based on spontaneous reports, it is not possible to assign an exact frequency to each of them.

Immune system disorders: allergic reaction (syncope, hypotension, dyspnoea, urticaria)

Skin and subcutaneous tissue disorders: blistering, discomfort/pain, swelling, erythema, irritation, desquamation/desquamation, pruritus, rash, stinging/burning

Eye disorders: blurred vision (see also section 4.4)

After application to large areas (more than 10% of the body surface) and/or long-term use (more than 2-4 weeks) or application under occlusive dressings, local skin changes such as skin atrophy, telangiectasias, excessive hair growth, stretch marks, hypopigmentation, secondary infection and acne-like symptoms may occur.

Notification of suspected adverse reactions

Post-marketing reporting of suspected adverse reactions is important. It allows continuous monitoring of the benefit-risk ratio of the drug. 4.9 Overdose

There are no reports of poisoning with Canesten HC. There is no risk of acute poisoning, as it is unlikely after a single dermal overdose (application to a large area under conditions favoring absorption) or after inadvertent oral ingestion. There is no specific antidote.

However, in case of accidental ingestion, gastric lavage is rarely necessary and should only be considered if a life-threatening amount of clotrimazole has been ingested in the last hour or if clinical signs of overdose (eg, dizziness, nausea or vomiting) occur. . ). ). Gastric lavage should only be performed if the airway can be adequately secured.

Action mechanism

Clotrimazole has an antifungal effect by inhibiting the synthesis of ergosterol. Inhibition of ergosterol synthesis leads to structural and functional damage to the fungal cytoplasmic membrane.

Primary resistance variants of susceptible fungal species are very rare. The development of secondary resistance in susceptible fungi has so far been observed only in very isolated cases under therapeutic conditions.

Hydrocortisone:

Hydrocortisone is a weak glucocorticoid and, to a lesser extent, mineralocorticoid corticosteroid. As an active ingredient of the topical cream, it has anti-inflammatory, anti-pruritic, anti-exudative and anti-allergic properties.

Hydrocortisone, like other topical glucocorticosteroids, has anti-inflammatory, anti-allergic, immunosuppressive, anti-mitotic (anti-proliferative), anti-pruritic and vasoconstrictive effects on the skin. In this way, in addition to the elimination of inflammation and itching, the keratinization process is normalized, the excessive activity of fibroblasts and epidermalization is inhibited, the pathological metabolic products are degraded and acantholysis is inhibited. However, this is not a curative treatment, but a symptomatic treatment.

5.2 Pharmacokinetic properties

clotrimazole:

Pharmacokinetic studies after topical application have shown that clotrimazole is poorly absorbed into the bloodstream through intact or inflamed skin. Peak serum concentrations of clotrimazole obtained were below the detection limit of 0.001 µg/ml, suggesting that topical clotrimazole is unlikely to result in measurable systemic effects or side effects.

Hydrocortisone:

The absorption of hydrocortisone through the skin depends on the thickness and condition of the skin. In normal skin, no systemic effects of corticosteroids have been observed after topical application.

However, for inflamed or damaged skin, skin absorption may increase depending on the application site, the use of occlusive dressings, the degree of skin damage and the size of the treated area. Under these conditions, systematic effects cannot be ruled out.

An increase in skin temperature or moisture, e.g. in skin folds or under an occlusive dressing, it also promotes absorption. In infants and young children, the epidermal "barrier" is still poorly developed, which facilitates transdermal absorption of drugs. The occurrence of systemic effects depends partly on the dose and more on the duration of treatment.

More than 90% of absorbed hydrocortisone is bound to plasma proteins. Hydrocortisone is metabolized in the liver and tissues and the metabolites are excreted in the urine. The biological half-life is about 100 minutes.

Significant absorption of hydrocortisone is not expected after short-term application to limited areas of inflamed skin.

5.3 Preclinical safety data

Non-clinical data reveal no special hazard for humans based on studies of repeated dose toxicity, genotoxicity and carcinogenicity. Clotrimazole was not teratogenic in reproductive toxicity studies in mice, rats and rabbits. In rats, high oral doses were associated with maternal toxicity, embryotoxicity, reduced fetal weight, and reduced pup survival.

In rats, clotrimazole and/or its metabolites were excreted in milk at higher concentrations than in plasma within 4 hours after administration and then decreased to a factor of 0.4 within 24 hours.

Hydrocortisone:

As an adrenocortical hormone, hydrocortisone is classified as relatively non-toxic for topical application. Teratogenic effects of high doses of corticosteroids, including cleft palate, growth retardation, and fetal lethality, have been observed in animal studies.

Clotrimazole plus hydrocortisone:

Non-clinical data reveal no special hazard for humans based on acute and repeated dose studies. In a 90-day multiple-dose dermal study, effects were observed only at exposures considered sufficiently greater than maximum human exposure, indicating little relevance to clinical use.
